Door prize contest details for the 2009 Montreal Health, fitness & lifestyle show.
Contestants must be 18 years of age or older as of January 1st 2009.
To be eligible for door prize drawing, contestants must have correctly completed the Health, Fitness and Lifestyle Show official entry form.
Entry forms are to be deposited at the Information Desk of the 2009 Montreal Health, Fitness and Lifestyle Show (Hall 400 East, Place Bonaventure, Montreal).
Contest begins at 3pm Friday 20th February 2009 and closes at 4pm Sunday 22nd February 2009. Deadline for entering the contest draw is 4pm Sunday 22nd February 2009.
Prizes range in value from $50 (taxes included) to $1 219 (taxes included).
Prize list is as follows:
Laser Dermamode gift cards
Value of $50 (taxes included) each. 10 gift cards.
Applicable to treatments received at Laser Dermamode.
Fitt & Fabulous Personal Training gift certificates
Value of $80 each (taxes included)
4 gift certificates.
Applicable for one personal training workout of 1 hour.
Rosibar and La fourmi biunique gift bag
Value of $100 (taxes included).
One gift bag that contains La fourmi bionique and Rosibar goods.
Club Sportif Côte-de-Liesse gift certificates
Value of $1,219 (taxes included).
Each
2 all-inclusive one year memberships at Club Sportif Côte-de-Liesse.
Respecterre gift certificate
Value of $100 (taxes included).
One gift certificate applicable for purchase(s) at Respecterre.

Value of $50 (taxes included).

Value of $677.25 (taxes included).

Value of $25 each (taxes included).
Gift certificates applicable against any purchase at a Liquid Nutrition Store.
Two (2) FASHION magazine gift subscriptions
Value of $14.62 (taxes included).

One year subscriptions to FASHION magazine.
Prizes are non-refundable and non-exchangeable for cash or its equivalent.
Eligible contestants can only complete one entry form per day.
Eligible contestants must provide a valid phone number, email and mailing address via which they can be reached so that they can be notified in the event that they are a winning contestant.
Winning entries will be selected (at random) and will be announced at 4:30pm on Sunday 22nd February 2009 at the main stage of the Health, Fitness and Lifestyle Show (Hall 400 East, Place Bonaventure, Montreal).
The winning contestants will be selected at random. Winning entry forms will not be returned to the lot from which winning contestants are being selected ie one prize per entry form only.
Winning contestants will be contacted by telephone and email (as well as mail if necessary). Prizes must be collected (by pre-scheduled appointment only) at the office of the Health, Fitness and Lifestyle Show (800 de la Gauchetiere West, Suite 8850, Montreal, Quebec, H5A 1K6). Appointments for collecting prizes will be given during normal working hours only (Monday to Friday, 9:00am to 5:00pm). Prizes will not be delivered. Deadline for collecting prizes is 30th April 2009.
In order to claim his prize, the winning contestant will be require to furnish two (2) pieces of government issued identification and will be required to answer the following skill testing question: (10 x 2) + 3 =
Winning contestant names will not be published.
Eligible contestants may contact the Montreal Health, Fitness and Lifestyle Show either by telephone (514) 397-2250 or email info@hflshow.com from February 23rd 2009 until 30th April 2009 in order to ascertain whether or not they are a winning contestant.
Employees, representative and agents of the Montreal Health, Fitness and Lifestyle Show, as well as persons with whom they are domiciled are not eligible for the prize drawings.
Any litigation respecting the conduct or organization of a publicity contest may be submitted to the Régie des alcohols, des courses et des jeux for a ruling. Any litigation respecting the awarding of a prize may be submitted to the Régie only for the purpose of helping the parties reach a settlement.
